Must bring previous immunization records
 TB Testing Monday and Tuesday: 9 AM - 4 PM
Wednesday: 9 AM - 12 PM
Testing fee: $17

Children's Immunizations Indiana requires that all children are current with the CDC recommended immunization schedule before entering kindergarten, 6th grade and 12th grade.
If you have no insurance or have Medicaid, you must bring your Medicaid card with you and you will qualify for State vaccine.

If you have private insurance, you must bring your card with you and we will process your insurance.
